| 課程簡介 | Parkinson’s disease (PD)-related falls can be expected to have a major impact on health care systems in the coming decades. Although the pathogenesis has multifactorial causation, balance and gait impairment is a major contributor. Besides balance and gait impairment, several risk factors had been reported for increasing risk of falls, including previous falls, older age, disease duration, advanced disease, cognitive function impairment, and presence of sarcopenia with low extremities weakness and autonomic dysfunction Therefore, PD-related falls can be expected to have a major impact on health care systems in the coming decades. Our present research has the preliminary conclusion was as follows: (1) Decreased stride length and speed are hallmarks of falls and highlighted the relationship among stride length and speed, clinical disease severity, and risk of falling. (2) Dopaminergic therapy can improve clinical functional scores but worsen balance-related measures. Increased sway length and velocity during the medication state are hallmarks of future falls, particularly in the advanced stage of PD. (3) Anticipatory postural adjustment (APA) is crucial for initiating a step. Higher APA duration and lower APA velocity in the mediolateral direction are not only surrogate markers reflecting the severity of freezing of gait but also reflect the fall risk. Finally, we applied Random Forest to the features selection and found the range of motion in the spine base angle and shoulder, elbow, hip, and knee angle on the most involved side is the major difference between patients with or without postural instability and gait disturbance phenotypes. |
